The colors used are Red Heart Super Saver: teal, hunter green, gold, carrot, black and red; and Caron Simply Soft: Sage.

 

As others suggested, I made each round of all 12 squares all at once. When I got to the outside black I quit following the pattern as it didn't make sense to me, but the idea was clear, to turn this motif into a square.
