It appears to have been published in a book, "Herrschners Crochet Afghans 2008 National Afghan Contest Winners".  I didn't see it on the Herrschners site, but you might do some googling to see if the book is on eBay or a used book site.

Thank you so much. I called Herrschners and they told me it was not longer available. I have tried to find the book online with no luck so far. Thank you for your response!

Be sure to check with your local library and also thrift stores. If your library does not have it, they can possibly borrow from another library. My local library has borrowed books for me even from out of state.
