I made some for my crochet Christmas gifts this year using inkjet printable fabric sheets. Make sure you use the kind that can be made colorfast - check the label carefully!

 

I use a cute picture of a cat playing with a ball of yarn and typed "Handmade by Mom for _____" in a nice font. I just printed, made colorfast by rinsing in cold water and ironing dry, and cut them out and sewed them in. The edges did not seem to ravel, althoug if it was an item you were going to wash alot, you might put fray check around the edge, zig-zag it on your sewing machine or turn the edges under.
