Could someone please help me to understand this pattern?

I just don't understand the Arm Hole part....I tried doing what I thought it meant, but I ended up with something that resembled a straight jacket! I tried doing it a different way, and ended up with something that resembled a huge frustrating mess! :bang :bang :bang

I'm sure it's me as I'm still struggling to understand and read some patterns, so if you could help me out, that would be wonderful!
