No. If directions say chain 4 you should chain 4. The loop on the hook isn't a chain. It is a part of your last stitch in the prev row. You must be doing treble stitches. They usually call for ch 4. 4 chains are needed to reach height of the stitches but sometimes you can reduce turning chains if less are needed to reach height.

:think Are you doing DC with a CH space to follow? That starts with CH 4 but maybe the instructions should say (CH 4 stands as 1DC and CH1.)

:hook As has already been said, the CH on the hook is never counted because without that, you could not do anything else.
