Ideas for adding tags to projects

I'm hoping someone here can help me. Before shipping my Christmas presents for family up north (will be doing it by the end of next month) I wanted to put a tag of some kind on each item so that years from now they could look back and remember that I made it for them. I was thinking the tag could say something like

"Made with love by Donna 2005" or something like that.

 

Question is, how would I go about making a tag like this and how would I attach it to the items. It would need to be small enough to not be noticeable in everyday wear, but something that isn't easily torn off and that will stay even after being washed.

I have always used those woven personalized tags from Herrshner's. Then you can add the date with a laundry marker.
