I had to get a hook and yarn for this one.  So, you've just finished the picot and your hook is on the left bottom of it.  I believe what it is telling you is to stabilize the stem of the picot.

 

Reach across the front of the picot's stem (toward the right) and around the back of it's stem, then pull up a loop thru the loop on your hook.  This will give the picot a slip stitch around the front of the stem of the picot, but the thread from the ball will be in back, on the right side.  When you make the next stitch, the stem of the picot will be (sort of) encircled.

 

I've made 3-lobed picots before and never done this.  I think you could either skip it if you want (or if I've confused you), or maybe stabilize it with just a slst into the 'front strand' of the original sc.
