Donation to Childrens hospital crochet toys what to write on gift tag?

I have been donating toys for some time now but haven't attached a tag as I don't know what to write, any ideas.

What kind of tag do you mean? Instructions for laundering and care of the item? A "to/from" tag?

I always attach a tag that tells who crocheted it, and the wash/dry instructions.  If sending a whole box of afghans/hats/whatever, I type up a nice cover letter introducing myself, with the information about who made them, and the laundering information.

Sample tag:  Crocheted with love and prayers by........

                      Machine wash on warm and machine dry on low
