Where to buy personalized labels?

Hi all! I would like to buy some personalized labels to put on the back of my special pieces. I've done a general search, but not really found what I was looking for. I figured some of you out there are already using them and would have advice on where or what to get.

 

I want it to say something along the lines of

Made with love by

Kristy Marriott

Alchemyfyr Designs

 

and I would also like to write the date on it.

 

Do you prefer sew on ones, are the iron on ones better?

Herrschner's has labels that you can order with your name on them. I don't know the price, but I didn't think they were too expensive.
