Help please

Ch 39. (Note: if you work into the “bumps” on the wrong side of the ch instead of into the front of the ch, you can ch 38 here and ch 4 instead of 5 on Rows 3 and 5.)
Row 1 (WS): Slip st in 2nd ch from hook and in next ch,
sc in next ch, hdc in next ch, dc in each of the next 34 ch, turn – 38 sts

not sure if I start a new piece or and it to where?

When you see a line of a pattern like the one you quoted, "row 1: "do stuff"--38 stitches" it means after "doing stuff" your stitch count should be 38--it's a sort of sanity check to make sure you have the right number of stitches at that point, not telling you to make 38 more stitches.
