Here is one but I think your's looks like a less than 1 year old size maybe?  This is for a 3-4 year old and somehow the proportions look odd to me.  There are no projects so I can't look at people's comments.  https://www.ravelry.com/patterns/library/hooded-cardigan-sweater-2  This doesn't have patch pockets but your photo's pockets are just sewn on rectangles.

I  will be back.  I'm sure there are a bunch more raglan cardigans out there that may not have a hood, and the hood is just a rectangle that can be attached to the cardigan in a couple of different ways.  Was there a specific size range you were looking for?

Here is one in an interesting pattern, but is based on a DC stitch so you could do in straight DC if you wanted.  No hood

Totally not like your pattern STITCH but it had a hood, and the stitch pattern is DC based.  You could omit the cables, I don't think it would affect the gauge in crochet as it would in knitting https://www.ravelry.com/patterns/library/hooded-cable-sweater  You have to create a sign on to freepatterns.com, but it's free (has Annie's Attic, Crochet! and Crochet World patterns for free, the patterns rotate so if you see one you like, download it for later).  

More on the  hood, you have a couple of patterns with hoods so if you wanted to use the pattern without the hood you could borrow the concept from the other patterns.  Both of the hooded patterns I linked, as well as your example, had the hood seam at the top of the head.  To make a hood like this, you basically work back and forth across the neckline a bit back from the front edge, like a really tall collar, then seam at the top (I'd use a mattress stitch so it's flat and less visible).
